I/We hereby apply for Registration as indicated in Item 1 Application Details.

I/we agree to provide copies of documents and other information or facilities requested by SSAIB relating to this application and consent to SSAIB carrying out any investigations including credit references necessary for the purposes of this application.

I/we hereby agree and undertake to be bound by and comply with the SSAIB Rules (available on the SSAIB Web-site) at all times whilst I/we remain a Registered Firm and (in respect of such of the SSAIB Rules which apply after cessation) at all time thereafter.

For the purpose of the Data Protection Act 2018, and generally, I/we hereby acknowledge and certify as follows:

• a) I/we acknowledge that the information and data set out in this form and all other information and data provided (or to be provided) in connection with this application for Registration is required by SSAIB in connection with, and for the purposes of the appraisal by SSAIB of an application for Registration or request for the continuation of Registration.
• b) I/we hereby consent to the processing by SSAIB of the information and data contained in this form for the purposes set out in paragraph a) above;
• c) I/we agree that SSAIB (or any agency or organisation appointed by it) may make any enquiries in relation to this information and data for the purposes of verifying it;
• d) I/we agree that SSAIB may disclose the information and data contained in this form for any purpose in connection with this application for Registration or in connection with any other application for Registration or request for the continuation of Registration.
• e) I/we accept that this application is only valid for 12 months after the application submission and note that a new application will be required after this date.
